The greatest opportunity to eliminate or minimise risk is during design, with later phases used to verify outcomes, resolve or transfer remaining risks, and capture lessons from the completed asset.

The DOOR framework runs across four phases, from setting a project up before design begins through to reviewing the completed asset in use. Each phase covers a distinct part of the lifecycle, and together they make Safety in Design continuous rather than a single event at one milestone.
Every phase is made up of stages. These break a phase into the actual pieces of work, giving clarity around who is responsible, when it happens, and what the phase has to produce before the next one starts. Formal Safety in Design reviews and workshops occur where required through the later phases, while Plan establishes the context, management approach, DOOR Plan and DOOR Register.
Stages act as checkpoints, and their order matters. In Plan, understanding the design context and DOOR management approach comes before setting up the DOOR Plan and DOOR Register. In Develop, preliminary design, interrogation, design update and design/product change form an iterative process until risks are eliminated or minimised so far as reasonably practicable.
In Validate, verification comes before transfer, so nothing is handed over that has not been checked against the built work. In Review and learn, findings are gathered before they are recorded, so lessons reach the next project instead of sitting in a closed register.

Review the completed outcome from a post-completion perspective (typically 8-12 months after completion), gather observations, feedback and other relevant inputs, and turn them into recorded findings and actions. The review includes physical inspection of accessible areas of the completed asset and should record any limitations to the inspection. Close or assign remaining items, capture risks, defects, deviations and opportunities, and make lessons available so future projects and design guidance can benefit.

The resulting lessons and recommendations can now inform future projects, design guidance and decisions rather than remaining isolated within the completed project.
